Rangely Family Medicine now offers an enhanced care management program for eligible Medicare patients through ChartSpan. This program adds an extra layer of support beyond your regular office visits—because we know health questions don’t always happen during business hours.

With ChartSpan, patients receive:

• Access to a 24/7 care line, 365 days a year
• Personalized care plans tailored to your health goals
• Help coordinating medication refills, appointments, and follow-ups
• Ongoing support to help reduce hospitalizations and manage chronic conditions

Annual Wellness Visits: Preventative Care at No Cost

In addition to ChartSpan, we also offer Medicare-paid Annual Wellness Visits (AWVs) using a convenient, tablet-based program. These visits focus on prevention, early detection, and long-term health planning—and they are completely free for eligible Medicare patients.

Annual Wellness Visits are different from a traditional physical. They help:

• Identify health risks early
• Prevent or manage chronic conditions
• Create or update a personalized prevention plan

Many patients can complete their Annual Wellness Visit during a regularly scheduled appointment, making it an easy extra step toward staying healthy.
