Who can nominate a staff member?

Anyone can submit a nomination—coworkers, physicians, patients, patient families, visitors, staff members, and administrators—who experiences or observes extraordinary and compassionate care being provided by a deserving RDH Nurse or Patient Care Technician. 

Please see both award opportunities below, followed by the online nomination submission form. 

Nurses can be nominated by anyone. In the healthcare setting, nominations come from patients, family members, other nurses, physicians, clinicians, or staff - who experience or observe extraordinarily compassionate care being provided by a nurse. In the academic setting, nominations are submitted by students, preceptors, faculty, staff, or alumni.

Nomination forms are available online at rangelyhospital.org or by contacting us at (970)675-5011. One of the keys to this program’s impact is the stories of compassionate care it brings to light. The nomination form at the bottom of the page has room for a story exemplifying why you think this Nurse should be considered for a DAISY Award.

How does RDH celebrate and recognize award winners?

One DAISY award recipient is honored annually at the “DAISY Day” ceremony. They receive a framed certificate, a DAISY Award signature lapel pin, and a stone sculpture from Zimbabwe, entitled "A Healer's Touch."

 The Petal Award is a way to recognize all the amazing patient care technicians we have at RDH. 

In conjunction with the Daisy Award, the RDH Foundation is spearheading another recognition program to honor all our patient care technicians.  The green lily represents purity, new beginnings, tranquility, and balance. All characteristics we strive for our staff to have. 

The Petal Award can be given to all Patient Care Technicians:

CNAs, MAs, EMTs, Laboratory, Respiratory, Radiology, Physical Therapy, Pharmacy Staff, etc. 

 All Petal Nominees will receive a copy of their nomination, a pin, a certificate, and will be honored during the Staff Recognition Awards hosted by the Rangely District Hospital Foundation.
